Produz grandes part\u00edculas de lipoprote\u00edna de muito baixa densidade (VLDL), enriquecidas com triglic\u00e9ridos e colesterol. Na corrente sangu\u00ednea, a enzima chamada lipoprote\u00edna lipase converte os triglic\u00e9ridos nas part\u00edculas de VLDL em \u00e1cidos gordos livres, tornando-os dispon\u00edveis como combust\u00edvel para tecidos perif\u00e9ricos, como m\u00fasculos e tecido adiposo. \u00c0 medida que esses triglic\u00e9ridos s\u00e3o libertados, a part\u00edcula de VLDL torna-se menor, transformando-se primeiro em lipoprote\u00edna de densidade interm\u00e9dia (IDL) e, em seguida, em lipoprote\u00edna de baixa densidade (LDL). Assim, o LDL \u00e9 o produto final do fornecimento de energia ao corpo atrav\u00e9s do metabolismo dos triglic\u00e9ridos e cont\u00e9m mais colesterol do que os triglic\u00e9ridos. Os receptores de LDL permitem que o f\u00edgado absorva a LDL na corrente sangu\u00ednea. Da mesma forma, os restos de quilom\u00edcrons, as part\u00edculas produzidas no intestino a partir dos triglic\u00e9ridos obtidos da dieta, passam pela mesma transforma\u00e7\u00e3o, sendo tamb\u00e9m absorvidos pelo f\u00edgado.      <\/p><p>O LDL \u00e9 uma pequena part\u00edcula que pode passar pelo endot\u00e9lio das art\u00e9rias, onde pode ser fagocitada pelos macr\u00f3fagos, resultando na forma\u00e7\u00e3o de c\u00e9lulas espumosas. A fisiopatologia da aterosclerose causada pelo LDL \u00e9 descrita no cap\u00edtulo <a href=\"\/atherosclerosis\/\">Fisiopatologia da aterosclerose<\/a>. <\/p><h2>Hist\u00f3ria do LDL<\/h2><p>A primeira evid\u00eancia da rela\u00e7\u00e3o causal entre o colesterol LDL plasm\u00e1tico e as doen\u00e7as cardiovasculares foi fornecida h\u00e1 mais de 100 anos. O m\u00e9dico e investigador russo chamado Anitschkow observou sinais de aterosclerose avan\u00e7ada nas art\u00e9rias de coelhos que foram alimentados com excesso de gema de ovo rica em colesterol. Atualmente, existem evid\u00eancias extensas e inequ\u00edvocas da rela\u00e7\u00e3o entre o colesterol LDL elevado e a aterosclerose com base em estudos observacionais, gen\u00e9ticos e em animais, bem como em ensaios cl\u00ednicos randomizados controlados, incluindo centenas de milhares de indiv\u00edduos. Al\u00e9m disso, grandes estudos randomizados controlados por placebo com medicamentos para baixar o colesterol LDL demonstraram que, durante um per\u00edodo m\u00e9dio de seguimento de 5 anos, cada redu\u00e7\u00e3o de 1 mmol\/L no colesterol LDL resulta numa redu\u00e7\u00e3o do risco relativo de doen\u00e7as cardiovasculares de aproximadamente 20%. letter-spacing: 0px;\">LDL e gen\u00e9tica<\/span><\/h2><p>Atualmente, tamb\u00e9m dispomos de dados gen\u00e9ticos sobre como o colesterol LDL est\u00e1 associado a doen\u00e7as cardiovasculares.A <a href=\"https:\/\/lipidtools.com\/pt-pt\/a-hipercolesterolemia-familiar\/\">hipercolesterolemia familiar (HF)<\/a>, por exemplo, \u00e9 uma doen\u00e7a autoss\u00f3mica codominante caracterizada por n\u00edveis muito elevados de colesterol LDL desde o nascimento, pelo que esses pacientes t\u00eam um risco elevado de doen\u00e7as cardiovasculares. Por outro lado, certas variantes gen\u00e9ticas que causam n\u00edveis mais baixos de colesterol LDL ao longo da vida resultam em menor incid\u00eancia de doen\u00e7as cardiovasculares. Como o material gen\u00e9tico (ADN) est\u00e1 definido \u00e0 nascen\u00e7a, havendo 50% de probabilidade de herdar um alelo do pai ou da m\u00e3e, esse tipo de pesquisa gen\u00e9tica tamb\u00e9m \u00e9 considerado um m\u00e9todo para demonstrar a causalidade entre o colesterol LDL elevado e as doen\u00e7as cardiovasculares. Esses tipos de estudos chamados de estudos de randomiza\u00e7\u00e3o mendeliana s\u00e3o an\u00e1logos aos ensaios cl\u00ednicos randomizados controlados com medicamentos.  Estudos de randomiza\u00e7\u00e3o mendeliana tamb\u00e9m demonstraram que a redu\u00e7\u00e3o de 1 mmol\/L no colesterol LDL geneticamente determinado durante um seguimento m\u00e9dio de 52 anos leva a uma redu\u00e7\u00e3o de 54% do risco de doen\u00e7as cardiovasculares.<\/p><p>Em resumo, a exposi\u00e7\u00e3o ao colesterol LDL ao longo da vida determina o risco de doen\u00e7as cardiovasculares.
